Service Facility Location Numbers Required As of January 1, 2008
As of January 1, 2008 the Service Facility Location number will be required for all CMS 1500 Professional claims (paper & electronic) for services rendered in the following locations:
- 21 - Inpatient Hospital
- 22 - Outpatient Hospital
- 23 - Emergency Room - Hospital
- 31 - Skilled Nursing Facility
- 32 - Nursing Facility
- 51 - Inpatient Psychiatric Facility
- 61 - Comprehensive Inpatient Rehabilitation Facility

For paper CMS 1500 submissions, you must use the service facility NPI in Block 32a and/or the two digit 1B qualifier along with the service facility legacy number in Block 32b. For electronic 837P submissions, in the Service Facility Location Secondary Identification REF segment, loop 2310D, NM1 you must use the XX qualifier and the service facility NPI and/or the 1B qualifier and service facility legacy number in the REF segment. |
